What is an ID5 Form?
An ID5 form is an HM Land Registry certificate used when the identity checks for an ID1 or ID2 form are carried out remotely by video call.
The certificate is completed by the conveyancer conducting the check. It provides HM Land Registry with evidence that the required identity verification has taken place remotely.
When is an ID5 Form required?
An ID5 may be required when you need to prove your identity for certain HM Land Registry applications and choose to complete the verification by video call rather than attending an appointment in person.
This can apply to a range of property matters, such as transferring property, changing ownership details or dealing with certain applications where HM Land Registry requires evidence of identity.
What Documents Will I Need?
The documents required will depend on whether your ID5 verification is being completed alongside an ID1 or ID2 form. You will usually need:
- Valid photo ID – such as a current passport or other accepted photographic identification.
- Proof of address – such as a recent bank statement, utility bill or other accepted document showing your current address.
- Your ID1 or ID2 details – depending on the HM Land Registry identity verification you are completing.

Who Completes an ID5 Form?
An ID5 form must be completed by an authorised conveyancer who has carried out the required identity checks. Once the required checks have been completed, the conveyancer will complete and sign the ID5 certificate to confirm that your identity has been verified remotely.
You do not need to complete the ID5 certificate yourself. Your role is to provide the required identification documents and attend the video appointment.
ID5 Verification Cost
There is no separate fee for an ID5 form, as it forms part of the remote verification process for your ID1 or ID2 form.
